Somebody sat with you in a hospital corridor, lent you money without asking why, or answered at two in the morning. It was probably never thanked for properly. This is that.

Continue writing opens that message in the card with the words already in it, so all that is left is to change a word and send it. The last message is the longest of the six, and it is the only one you have to copy across yourself.
Thank you for last month. I don't think I said it properly at the time, and it's been on my mind since. You showed up without being asked and stayed a lot longer than you needed to. I noticed all of it.

Thank them for the specific thing, including the awkward part. What lands is not thanks for everything, it is thank you for driving to the hospital at 11pm and then not making it a story afterwards. Naming what it cost them is what turns a courtesy into a real acknowledgement.
Do not settle the debt in the card. I owe you one turns a gift into a ledger and lets you both off the hook. Just say thank you, fully, and repay it later without announcing that it is repayment.
Say it once, in writing, so it stops being the thing you keep meaning to say.
